Speaker Registration Form

A speaker registration form must be completed for ALL people presenting a session at the conference.
Even if you are only attending to present your session, we need you to register for name badge and catering purposes.
If you have already completed a general registration form, please complete and submit this form. (We won't invoice you again!)

Your Details

Please record your name as you wish it to appear on your registration name badge.
This is the address that we will use to send your conference correspondence and payment receipt.
This is the email address we will use to communicate with you about your registration and the conference.
We will use this phone number to contact you if there are any problems with your registration.

Your Presentation

If you are presenting in multiple sessions, please enter a full stop after each session number (eg., 1.13.26 would mean you are presenting in sessions 1, 13 and 26)

Registration Details

If there are multiple people in your presentation, please ensure that you have discussed who will be accepting the free conference registration.
Dinner is NOT included in the free conference registration.

Payments must be received within 48 hours of receiving an invoice.
Payments can be made by direct deposit, internet banking or bank transfer to "TKWMP 2019 Conference" Bank Account # 02-0704-0121185-001.
A receipt will be emailed to the email address you have provided once payment has been processed.
